Instead of treating “mind” and “body” separately, BHI brings therapists, primary care providers, and care managers together to create one cohesive plan. This care process would reduce waiting rooms and procedure repetition because behavioral health providers would concentrate on comprehensive well-being evaluation and treatment.
The Behavioral Health Integration Program coordinates healthcare services so your primary care provider can adjust insulin doses while your therapist treats mood changes, all in sync. Monthly check-ins track progress, tweak treatments, and catch issues early so small struggles do not become significant setbacks.
Your providers start with a detailed behavioral health assessment to understand your personalized struggles. They use evidence-based tools to identify needs, such as dedicated screenings for depression or anxiety severity.
Patients will receive a proper care plan that addresses therapy, lifestyle changes, medication (if needed), and specialist consultations. A patient with both conditions could receive a combined care plan that includes diabetes glucose monitoring and weekly behavioral health treatment.
Your behavioral health care manager checks in monthly to review progress, update goals, and address setbacks. If treatments are not working, the team adjusts quickly, such as switching treatment methods or connecting you with a psychiatrist.
CoCM is an enhanced form of BHI involving a team-based mental health care approach to obtain better outcomes.
